Required Documents

  • Power of attorney duly authenticated by a notary and application issued by an attorney.
  • Complete copy of the passport, duly authenticated by a notary.
  • Criminal record certification, duly authenticated or apostilled.
  • Certificate of good health.
  • Sworn statement of personal background.
  • Proof of address: notarized lease agreement, with a copy of the landlord’s ID, or utility bill.
  • Two (2) passport-size photographs.
  • Copy of the migrant labor registry requested before MITRADEL.
